NOMINATION FORM FOR PRIMARY CITY TEAM 2011-12
Important Information
All boys nominated must be current year 5. i.e. year 6 in September 2011
Trial datesfor your school are on the accompanying sheet
If these clash with other school activities we will make give alternate dates so all boys have their opportunity
Boys at Academies and Centres of Excellence can be nominated
Teachers do not need to accompany the boys they have nominated; however there should be someone responsible to take the boy(s) home after the trials
All boys should wear football kit including shin pads.
Boys will not be allowed to participate without shin pads.
